Objective: Gastric cancer in individuals under 40 years old is uncommon and presents etiopathogenic aspects that have not yet been elucidated. This study describes the clinicopathological characteristics and prognosis of a series of young patients with gastric cancer. Material and Methods: This retrospective study was conducted at San Borja Arriarán Clinical Hospital. The digestive surgery database and the Diagnosis-Related Group (DRG) database were used to identify patients under 40 years old with gastric cancer during the period from January 2012 to April 2024. Clinical, pathological, and survival variables were analyzed. Results: Twenty-five cases were evaluated. The most frequent age at presentation was 39 years (20%). The predominant histological type was poorly cohesive adenocarci-noma with signet-ring cells (64%). The most common anatomical location was the middle third of the stomach (44%). The most frequent endoscopic findings were advanced Borrmann type III and type IV lesions (64%). Information on Helicobacter pylori infection was available for 15 patients (60%), with infection present in 53.3% of these cases. Stage IV was the most common (64%). Conclusions: In this series of young patients with gastric cancer, there was a high prevalence of advanced Borrmann type III or IV lesions, with a predominance of poorly cohesive adenocarcinoma with signet-ring cells. Most cases were diagnosed at stage IV. These findings underscore the need for early diagnosis in this population.
