Introduction: It is estimated that approximately 20% of patients with Sjögren's syndrome present neurological involvement such as polyneuropathy, transverse myelitis, and optic neuritis, unilateral or bilateral, among other manifestations. We characterize a series of cases of Sjögren's syndrome with central and peripheral nervous system involvement. Methods: Seven cases of patients between 28 and 64 years old were analyzed by the Hospital Barros Luco Trudeau neurologists using laboratory tests (rheumatic serology) and images (Magnetic Resonance Imaging) of diagnostic confirmation, between 2012 and 2024. Results: The average age was 43,7 years; six were women. Only two had Sjögren's syndrome before their evaluation. Four patients developed optic neuritis; two presented with transverse myelitis, one case presented as polyneuropathy, another presented as unmediated encephalitis, and the other as cerebrovascular disease. Clinical criteria and serology confirmed all cases. Conclusion: Among the neurological manifestations of Sjögren's syndrome, transverse myelitis and optic neuritis stand out as presenting symptoms. Encephalitis and cerebrovascular disease are an infrequent presentation of the illness.
