Avoidant/Restrictive Food Intake Disorder (ARFID) is defined as a lack of interest in eating or avoidance due to the organoleptic characteristics of food, preventing the fulfillment of nutritional needs. Only 15% of cases occur in adults, with a single case reported after bariatric surgery. Clinical case: A 33-year-old woman with a history of bariatric surgery, presented with abdominal pain, self-induced vomiting, and postprandial anxiety attacks at the third postoperative month, leading to hospitalization and ruling out organic causes. During outpatient follow-up, she continued to experience anxiety attacks and fear of pain, losing weight despite the intention to eat, without body dysmorphia or emotional dysregulation, leading to a diagnosis of ARFID. She spent 5 months in psychiatric ICU for nutritional management, anxiolytic treatment, and cognitive-behavioral therapy. She continued outpatient care, successfully reintegrating food and social life by the fourth month, and was subsequently discharged. Discussion: Studies link gastrointestinal disorders with ARFID symptoms in up to 21% of cases. Factors such as delayed digestion, constipation, and abdominal discomfort can exacerbate the fear of eating and avoidance, suggesting a potential link with bariatric surgery. Additionally, gastric bypass has been associated with increased rates of anxiety and depression, which also play a role in the manifestation and persistence of ARFID symptoms.
