Objective: San Borja Arriarán Clinical Hospital (HCSBA) was the first public hospital in Chile to adopt the Da Vinci® robotic surgery system. This study aims to characterize the treated population, describe the clinical outcomes of the surgeries, and analyze their costs. Materials and Methods: A descriptive data-base including clinical and economic variables of all robotic surgeries performed between October 2022 and October 2023 across 7 surgical specialties was analyzed. Quantitative variables were described using mean and standard deviation, and qualitative variables through frequencies and percentages. Results: A total of 186 surgeries were analyzed, involving patients with an average age of 57.98 (SD: 20.31) years. At least one comorbidity was observed in 74.19% of patients, with hypertension and type 2 diabetes mellitus being the most frequent. Cardiac surgeries exhibited the highest percentage of intra (29%) and postoperative (43%) complications, the highest volume of intraoperative bleeding (971 ml, SD: 690.6), the longest duration (180.57 min, SD: 56.9), more days of hospitalization (8.29 days, SD: 2.3), and more days of medical leave (25 days, SD: 3.3). The average cost of surgeries ranged from $2.9 to $4.7 million Chilean pesos (CLP), with cardiac and urological surgeries being the most expensive (average of $4.7 and $3.7 million CLP, respectively). Discussion and Conclusion: The implementation and use of robotic surgery in HCSBA could reduce costs at various levels, thus cost-effectiveness studies are required to compare robotic surgery with other techniques.
