Introduction: Gallbladder paraganglioma is an unexpected histological finding. Only 24 cases reports have been found in the literature. Clinical Case: Healthy female patient, 56 years old. Laparoscopic cholecystectomy was performed because of symptomatic cholelithiasis, without incident. Biopsy reports paraganglioma in the gallbladder. Discussion: Gallbladder paragangliomas are a rare finding. There are no guidelines for the management and surveillance of these incidentalomas. After reviewing the literature, there are no reports of postoperative complications or guidelines regarding study and follow-up.
