Introduction: Among the severe and rare complications of acute diverticulitis (AD) there is pylephlebitis, defined as septic thrombosis of the portal vein and/or its branches. Currently there are no clinical guidelines or consensus regarding its treatment. Aim: To carry out an analysis of patients with the diagnosis of pylephlebitis secondary to complicated AD hospitalized in the last year at the Clinical Hospital of the University of Chile (HCUCH), with emphasis on management. Materials and Methods: Descriptive and retrospective observational study of patients with the diagnosis of acute diverticulitis with associated pylephlebitis hospitalized at the HCUCH from October 2022 to October 2023. Simple and descriptive sta-tistics were used for the analysis. Results: All 9 patients included were managed with combined treatment (antibiotics and anticoagulation), achieving good results. At discharge, the duration of treatment was variable. Discussion: the basis of pylephlebitis treatment is combined therapy, however it is not standar-dized. There are no specific surgical indications for AD in the context of pylephlebitis, nor the indication for subsequent elective surgery.
