Chest wall contouring surgery is one of the first surgical interventions in transgender individuals undergoing female-to-male gender transition. The primary objective of this procedure is to facilitate the new gender role affirmation by masculinizing the thoracic region and eradicating the pre-existing feminine contour. Achieving an aesthetically satisfactory outcome that aligns with the new gender identity remains a critical challenge, making the surgical technique essential in accomplishing this goal. Various alternatives have been proposed concerning the shape, length, and positioning of both the scar and the nipple-areolar complex (NAC), the latter being a pivotal element in this process. The size and position of the nipple and areola differ between males and females. Typically, males possess an NAC with smaller dimensions and projection, as well as a more lateral positioning on the chest wall. This study aims to present a modification of the surgical technique initially described by Lo Russo8.
