Objective: To present the cross leg fasciocutaneuos flaps for coberture of the open fractures of the distal portion of the leg. Introduction: Open fractures of the distal portion of the leg need coberture with flaps. The most suitable flap is with microsurgery but if there is no such technique or the is no receptor vessels the cross leg fasciocutaneous flaps are a good solution. Material and Method: Eleven patients with open fracture of the distal portion of the leg were operated on the Salvador Hospital between 2011 and 2020 with the cross leg fasciocutaneous flaps. There were 10 men and one woman. Age was between 20 and 43 years old. Technique: From the posterior part of the contra lateral leg of the fracture one fasciocuta-neous flap was raised and sutured to the open fracture. This flap was inset in that place of fracture for 21 days. At the end of that time the cross leg flap was divided, so the fracture was covered completely with the distal portion of the contra lateral fasciocutaneous flap. Result: All flaps survive whithout necrosis or dehiscence. Celulitis in three patients was presented and treated with antibiotics. Discussion: The cross leg fasciocutaneous flaps is a good alternative to the microsurgery in open fractures of the distal portion of the leg. Conclusión: Cross leg fasciocutaneous flaps are reliable and sure in the treatment of the open fractures of the distal portion of the leg.
