| Acknowledgements. I have never been in debt to so many people and institutions. This book corresponds to an updated and synthesised version of my doctoral thesis defended at the University of Oxford, under the supervision of Leigh A. Payne, and evaluated by Alan Angell, Heather Hamill, and Brian Loveman.
